Countrywide offers eligible employees a comprehensive benefits package, including medical, dental, vision, prescription drug, life, and disability coverage. Our benefits program is available to eligible full-time employees, with coverage beginning on the first day of the calendar month following one month of employment. (Regular part-time employees scheduled to work at least 32 hours a week can participate in the medical, dental and vision plans, as well as the Flexible Spending Accounts.) Countrywide and its eligible employees share the cost of the medical and dental plans and employees pay the cost of the vision plan.
Medical Options
Countrywide UHC EPO Plan: For this plan, an eligible employee must use network providers. Eligible expenses are generally reimbursed at 80% after the deductible is satisfied.
Countrywide Aetna Choice PPO and Basic PPO: This plan allows eligible employees to choose any provider but pays a higher benefit if a network provider is used. The Choice PPO generally pays 80% of the eligible medical expenses after the deductible for in-network services and 60% for out-of-network services after the deductible.
Countrywide Definity Silver Plan: Eligible expenses are first reimbursed by a company-funded Health Reimbursement Account. If the account is fully spent, a deductible is applied. After the deductible, expenses are generally reimbursed at 80% for in-network and 60% for out-of-network services.
Countrywide Aetna Network-Only Plan: This plan is designed for those who do not expect to use a significant amount of healthcare but still want protection against large medical expenses. You must use network providers under this plan. There are copays for doctor office visits and hospitalization and the plan pays most other expenses at 60% after the annual deductible.
The Countrywide Aetna HSA Compliant Plan: This plan allows eligible employees to set aside their own funds in a tax-deferred Health Savings Account. They can use the funds for immediate or future healthcare needs. After the deductible is satisfied, the plan pays 90% of most eligible charges if a network provider is used and 70% for out-of-network services.
Regional Plans: In some locations, regional HMO, PPO and POS plans are available.
Countrywide Out-of-Area Plan: An indemnity plan is provided for those otherwise eligible employees who may live in an area where no physician network is available.
Prescription Drug Coverage: All plans provide for prescription drug coverage. A coinsurance or co-payment is required.
Dental Options
PPO Plans: Two PPO plans are available that pay higher benefits if the eligible employee uses network providers. Both plans pay a percentage of eligible charges after the annual deductible is satisfied. In addition, both plans include orthodontic benefits for adults and children.
DMO Plan: The DMO (Dental Maintenance Organization) Plan requires the use of a DMO network dentist. All preventive care and many basic services are paid at 100%. Some basic services and all major services and orthodontia require a co-payment.
Vision Care Coverage
Benefits are provided for an annual vision exam, annual corrective lenses or contacts and frames every 24 months after a copay.
Life Insurance
Basic Life & Accidental Death & Dismemberment (AD&D): Countrywide provides group term life and AD&D insurance coverage at no cost. The benefit level for life insurance is equal to one times the eligible employee’s annual base benefits rate, up to $100,000.
Supplemental Life Insurance: This plan provides eligible employees with the opportunity to purchase voluntary life insurance coverage for themselves, their spouses/domestic partners and children.
Disability Insurance
Short-Term Disability (STD): Countrywide provides basic STD coverage at no cost. The benefit provides up to 60% of the eligible employee’s base benefit rate, up to a maximum of $2,308 per week. The elimination period is 60 days.
Voluntary STD: This plan allows an eligible employee to purchase added STD protection. The plan is identical to the Company-paid program, except it has a 14-day elimination period.
Long-Term Disability (LTD): Countrywide provides eligible employees with LTD insurance at no cost. Benefits begin after 180 days of continuous total disability. The plan pays up to 60% of base benefit rate monthly earnings, to a monthly maximum of $15,000.
Flexible Spending Accounts
These plans allow eligible employees to set aside pre-tax dollars for eligible medical and dependent care expenses.
